Job summary

SnappyCX is seeking a Clinical Care Coordinator to remotely support a U.S.-based healthcare practice. The role requires a clinical background and strong care coordination skills to connect patients, providers, and insurers, ensuring timely, well-coordinated care.

You will handle patient intake, document interactions in the EHR/EMR, verify insurance, obtain prior authorizations, and coordinate referrals. This fully remote, independent contractor position follows US business hours.

Qualifications

Clinical background required; candidates with training or professional experience as a Nurse, Doctor, Physician Assistant, Medical Assistant, or other healthcare professional are encouraged to apply.
Strong understanding of medical terminology and clinical workflows.
Experience in care coordination, patient coordination, medical virtual assistance, clinical administration, or a similar healthcare role is strongly preferred.
Experience supporting U.S.-based healthcare practices is highly preferred.
Familiarity with EHR/EMR systems.
Experience with insurance verification, prior authorizations, referrals, or care management is an advantage.
Strong English communication skills; bilingual English/Spanish candidates are highly preferred.
Excellent patient communication, organization, documentation, and follow-up skills.
Comfortable communicating with patients, healthcare providers, insurance companies, and other care team members.
Ability to work independently in a remote environment while managing multiple patient needs and priorities.
Understanding of HIPAA and patient confidentiality requirements.

Responsibilities

Communicate with patients regarding appointments, treatment plans, follow-ups, referrals, and other care needs.
Coordinate patient care between providers, specialists, facilities, pharmacies, and other healthcare professionals.
Assist with patient intake and maintain accurate patient information within the EHR/EMR.
Document patient interactions, clinical information, and provider instructions accurately.
Coordinate and follow up on referrals and specialist appointments.
Assist with insurance verification, eligibility checks, and prior authorizations.
Follow up on outstanding medical records, laboratory results, imaging, referrals, and other clinical documentation.
Conduct patient follow-up and care management calls, including CCM calls when required.
Review patient information and identify issues that require escalation to the appropriate clinical provider.
Communicate provider notes, instructions, and other relevant information to patients.
Support prescription/refill requests and other clinical administrative workflows as appropriate.
Maintain accurate, confidential, and HIPAA-compliant patient records.
Provide compassionate and professional patient support while helping patients navigate their care journey.
