Seamless Assist is seeking a Remote Patient Care Coordinator for a US Cardiology team. The role focuses on patient communication, scheduling, care coordination, and clinical documentation, supporting a high-volume caseload in a remote, global setting.

The ideal candidate brings a clinical background (CNA/MA/RN), telehealth experience, and comfort managing 150+ patients with strong English communication. Working US hours is required, with HIPAA-compliant processes and EMR/EHR proficiency.

Qualifications

• Clinical healthcare background: CNA, MA, RN, or equivalent credential.
• 3+ years of healthcare, telehealth, or medical office experience.
• Experience in care coordination or care management, preferably telephonic or remote.
• B2+ English proficiency; assessed at screening.
• Experience using EMR/EHR systems.
• Strong patient-centered communication.
• Ability to work US Central Time hours consistently.
• Strong written and verbal English communication; assessed at screening.

Responsibilities

• Manage patient scheduling, reminders, and follow-up workflows in EMR/EHR systems.
• Conduct telephonic or remote patient outreach to reinforce care instructions and coordinate next steps.
• Support patients navigating cardiology care pathways, including CHF management protocols.
• Track testing schedules, follow-up appointments, and remote patient monitoring workflows.
• Coordinate with the clinical team to ensure continuity of care across a caseload of 150+ patients.
• Escalate clinical concerns to the appropriate provider within scope (no diagnosing or prescribing).
• Maintain accurate and timely clinical documentation in EMR/EHR systems.
• Ensure HIPAA compliance in all patient interactions and records.
• Support billing documentation and insurance coordination as needed.
