Job summary

The Lead Tree - Clark Outsourcing in Clark Pampanga is seeking a meticulous Medical Scheduler to coordinate patient visits for our Hospital at Home program. You will manage referrals, schedule nurses and therapy staff, and maintain precise records while acting as the liaison between hospital teams, patients, and clinicians.

You will grow with the program, supporting home care scheduling and after-hours coverage.

Qualifications

• Proven experience in scheduling, coordination, or a similar role, preferably in healthcare or homecare.
• Excellent organizational and multitasking abilities for a fast-paced environment.
• Strong communication and empathy when assisting patients and families.

Responsibilities

• Coordinate patient visits and staff assignments for the Hospital at Home program.
• Match clients with caregivers or nurses based on needs, skills, and availability.
• Maintain accurate patient records and update scheduling changes in systems.
• Follow up with vendors and clinicians to secure orders and authorizations.
• Create and adjust schedules for clients, nurses, and caregivers as needed.
