Job summary

Placements24 is seeking a compassionate Client Support Officer to join the team in Welkom. The role focuses on direct assistance to individuals and families, guiding them through social services and challenges, and coordinating support interventions to ensure high quality care.

Key duties include intake interviews, needs assessments, and developing individualized support plans with clients while maintaining confidential case files and liaising with partner agencies to optimize care outcomes.

Qualifications

• Diploma or Degree in Social Work, Human Services, or related field.
• Proven experience in client support, case management, or similar role.
• Strong understanding of community services and social welfare systems in Welkom area.
• Excellent communication, empathy and problem-solving skills.

Responsibilities

• Conduct initial client interviews and assessments to identify needs and eligibility for services.
• Develop and implement individualized support plans in collaboration with clients.
• Provide guidance and support to clients in accessing community resources, benefits, and programs.
• Maintain accurate and confidential client case files, documenting all interactions and progress.
• Liaise with other service providers, agencies, and stakeholders to coordinate client care.
• Offer emotional support and counseling to clients facing personal or social difficulties.
