Job summary

Praxis Care is seeking a Support Worker to empower individuals in independent living and to follow person-centred plans. You will work with Team Leaders to implement tailored support programmes, improving daily living skills and safety within the service.

The role requires flexibility in working hours and adherence to medication policies, with opportunities for training and career development within Praxis Care.

Qualifications

• Numeracy and literacy skills required.
• Effective communication skills essential.
• Experience in health and social care is desirable.

Responsibilities

• Support individuals to develop independent living skills per person-centred plans.
• Work with the team to implement individual and group support programmes.
• Promote a safe environment and handle challenging behaviours appropriately.
• Ensure medications are managed in line with policy where needed.
• Record information per service reporting guidelines.
• Attend required training and follow service policies and Health & Safety procedures.
