Job summary

Praxis Care seeks a dedicated Support Worker to empower individuals in independent living within a compassionate team. The role encompasses implementing person-centred plans and promoting a safe, respectful environment across shifts.

As a valued member, you will follow policies, participate in training, and support clients with diverse needs, including potential sleep-ins with premium night rate. A flexible pattern may apply across full-time, part-time, or relief work.

Qualifications

• Attention to detail and effective communication skills are essential.
• Experience working within health or social care is desirable.
• Full valid ROI/UK/EU driving licence and access to own vehicle.

Responsibilities

• Support and empower individuals to develop or improve independent living skills as identified in person-centred plans.
• Work with Team Leaders to prepare and implement individual and group support programmes.
• Collaborate with other Support Workers to promote a safe environment and resolve conflicts using appropriate techniques.
• Ensure medication is stored and administered per policy where necessary.
• Record and log information per service reporting guidelines.
• Attend all related training and follow health and safety policies.
