Job summary

Praxis Care is seeking a Relief Support Worker at Swan House to deliver high-quality care within a person-centred framework. You will support individuals to develop independent living skills and participate in planning with Team Leaders to implement tailored programmes.

Role involves maintaining safety, adhering to medication policies where required, and keeping accurate records. Flexibility to meet service needs and readiness to undertake related training are essential.

Qualifications

• Numeracy and literacy skills.
• Effective communication skills.
• Experience in health and social care field (desirable).

Responsibilities

• Support and empower individuals to develop or improve independent living skills as identified in person-centred plans.
• Prepare and implement individual and group support programmes with Team Leaders.
• Promote a safe working and living environment and manage conflict using appropriate techniques.
• Ensure medication is stored and administered per policy where necessary.
• Record and log information according to service reporting guidelines.
• Attend all related Health & Safety training and follow policies.
