Job summary

Praxis Care is seeking a part-time Support Worker to join a dedicated team, working 19.5 hours per week with pro rata pay. You will support individuals to live independently, follow person-centred plans, and collaborate with Team Leaders to deliver quality care within a respectful, dignified service.

The role requires flexible hours, driving licence is preferred, and the ability to follow policies and procedures while ensuring safety and well-being of clients.

Qualifications

• Numeracy and literacy skills.
• Effective communication skills.
• Desirable: experience in health and social care.

Responsibilities

• Support individuals to develop independent living skills per person-centred plans.
• Prepare and implement individual and group support programmes with Team Leaders.
• Promote a safe environment and resolve conflicts using appropriate techniques.
• Ensure medication is stored and administered per policy when required.
• Record information according to service reporting guidelines.
• Follow Health & Safety policies and participate in training.
