Job summary

The Abbeyglen service under Praxis Care seeks a Team Leader to guide a team of Support Workers in delivering person-centred care. You will design individual support plans, coordinate service delivery, and ensure safety, dignity and independence for the people we support.

You will lead on assessments, staff supervision and regulatory compliance, working closely with families and multidisciplinary teams to promote quality of life and community integration.

Qualifications

• Essential: 2 years’ experience in a health and social care setting and QQI Level 5 in Health Care or equivalent.
• Desirable: Experience in mental health or learning disability services.

Responsibilities

• Plan, coordinate, and implement the safe transition of new individuals into the service and community where necessary.
• Liaise with families and multi-disciplinary teams to promote needs.
• Plan, develop, and implement individual and group programmes of support.
• Coordinate ongoing assessment and review through appropriate assessment models.
• Support physical, emotional, and personal needs with respect, privacy, dignity and independence.
• Ensure medication is stored and administered according to policy.
• Provide leadership, supervision, and management to Support Workers including attendance, capability, and disciplinary matters.
• Participate in development opportunities for staff and self.
• Ensure records and reports are produced in line with policy and statutory requirements.
• Support regulatory compliance with governing bodies.
