Job summary

Praxis Care is seeking a Team Leader to join our service in Cork. The role focuses on leading a team of Support Workers to design and deliver individual support programmes, ensuring the physical, emotional and personal needs of the people we support are met with dignity and respect.

You will work within a multi-disciplinary setting, coordinate care plans, maintain records, and support regulatory compliance while upholding Praxis Care’s policies and values.

Qualifications

• 2 years’ experience in a health and social care setting.
• Strong communication and leadership abilities.
• Full valid driving licence and access to own vehicle.

Responsibilities

• Plan, coordinate, and implement safe transition of new individuals into the service and community.
• Liaise with families and multidisciplinary teams to promote needs.
• Develop and implement individual and group programmes of support.
• Coordinate ongoing assessment and review using appropriate models.
• Support physical, emotional and personal needs of people we support.
• Ensure medication is stored and administered per policy and procedures.
• Provide leadership and supervision to Support Workers including attendance and capability procedures.
• Maintain necessary records and prepare reports in line with policy.

Education

QQI Level 5 in Health Care or equivalent
