Job summary

Praxis Care is seeking a Team Leader to oversee support teams for individuals in our service. You will design individual support plans and coordinate delivery by Support Workers, ensuring high standards of care and regulatory compliance.

The role requires 2 years in health and social care with QQI Level 5 and a valid driving licence. Competitive salary and benefits are provided, with pro rata adjustments for contracted hours.

Qualifications

• 2 years’ experience in a health and social care setting or equivalent.
• QQI Level 5 in Health Care or equivalent.

Responsibilities

• Plan, coordinate, and implement the safe transition of new individuals into the service and community.
• Liaise with families and multi-disciplinary teams to promote needs.
• Develop and implement individual and group programmes of support.
• Coordinate ongoing assessment and reviews using suitable assessment models.
• Provide leadership, supervision, and management to Support Workers.
