Job summary

Praxis Care is seeking a Team Leader to oversee a team of Support Workers, coordinating individual support plans and ensuring high-quality care. The role emphasizes leadership, safeguarding, and effective service delivery within a supportive organisational framework.

The position requires a minimum of 2 years in health and social care and QQI Level 5 qualification, with a valid driving licence and strong communication skills. A competitive salary and flexible working hours are offered.

Qualifications

• 2 years’ experience in a health and social care setting.
• Minimum QQI Level 5 in Health Care or equivalent.
• Right to Work in the Republic of Ireland.

Responsibilities

• Plan, coordinate, and implement the safe transition of new individuals into the service and community.
• Liaise with families and multi-disciplinary teams to promote needs.
• Develop and implement individual and group support programmes.
• Coordinate ongoing assessment and regular reviews.
• Support physical, emotional, and personal needs with respect, privacy, dignity, fulfilment, independence and choice.
• Ensure medication is stored and administered per policy and procedures.
• Provide leadership, supervision, and management to Support Workers.
• Maintain necessary records and produce reports per policy.
• Support the manager with regulatory compliance.
