Job summary

Praxis Care is recruiting a Team Leader to lead a support team and design individual support programmes for people in our care. You will coordinate a group of Support Workers and yourself to deliver person-centred care.

The role requires leadership, guidance of staff and adherence to Medication Policy and regulatory requirements. A driving licence and access to a vehicle are preferred to enable mobility across the service locations in Ireland.

Qualifications

• 2 years’ experience in a health and social care setting.
• Minimum QQI Level 5 in Health Care or equivalent.

Responsibilities

• Plan, coordinate and implement safe transition of new individuals into the service and community.
• Liaise with families and multi-disciplinary teams to promote the needs of the people we support.
• Plan, develop and implement individual and group programmes of support to enhance independence.
• Provide effective leadership, supervision and management to Support Workers.
