Job summary

Praxis Care is seeking a Relief Team Leader for Windermere to oversee a team delivering person-centred support, ensuring safety, dignity, and independence for individuals in the service. The role involves coordinating plans, liaising with families and multidisciplinary partners, and managing records and regulatory compliance.

The ideal candidate has at least 2 years in health and social care, QQI Level 5 in Health Care, and a valid driving licence.

Qualifications

• 2 years’ experience in a health and social care setting.
• QQI Level 5 in Health Care or equivalent.
• Full valid driving licence and access to own vehicle.
• Right to Work in the Republic of Ireland.

Responsibilities

• Plan, coordinate, and implement the safe transition of new individuals into the service and community as needed.
• Liaise with families and multidisciplinary teams, community groups, and voluntary bodies to promote needs.
• Plan, develop, and implement individual and group programmes of support to enhance independence.
• Coordinate ongoing assessment and regularly review with suitable assessment models.
• Support physical, emotional, and personal needs with respect, privacy and dignity.
• Ensure medication is stored, held, and administered per policy and law.
• Provide leadership and management to Support Workers, monitor time/attendance, and participate in disciplinary procedures.
• Engage in learning and development and identify personal development opportunities.
• Maintain records and produce reports per policy and statutory requirements.
• Support manager with regulatory compliance.
