Praxis Care is seeking a Relief Team Leader at Iona House, Castleblaney, Co. Mayo. The role focuses on leading a team to meet practical and emotional needs, coordinating care plans, and ensuring adherence to medication policies and regulatory requirements.

Applicants should have health care experience and relevant QQI Level 5 certification. The post requires driving and flexibility to meet service needs, with opportunities for professional development and supervision within a supportive care

Qualifikationen

• Minimum 2 years’ experience in health and social care.
• QQI Level 5 Health Care or equivalent.
• Full valid driving licence and access to own vehicle.
• Experience in mental health or learning disability services is desirable.

Aufgaben

• Plan, coordinate, and implement safe transition of new individuals into the service and community.
• Liaise with families and multi-disciplinary teams to promote needs.
• Plan and implement individual and group programmes of support to enhance independence.
• Coordinate ongoing assessment and regular reviews.
• Support physical, emotional, and personal needs with respect, dignity and independence.
• Ensure medication storage and administration per policy and law.
• Provide leadership, supervision, and management to Support Workers.
• Engage in learning and development and address development needs.
• Create and maintain necessary records and reports per policy.
• Support regulatory compliance with all regulating bodies.
