To lead the Healthy Families Team in the delivery of the 0-19 Healthy Child Programme and Public Health Nursing Programme Children in line with local and national public health priorities.

1 full time post based in Ashfield.

We are a quality integrated service working in partnership with local Family Hubs, working with children, young people aged 0–19 and their families.

The team is a skill mixed team, taking leadership from Specialist Public Health Practitioners, providing a high standard of care. Our staff receive good training and an induction package and a friendly supportive working environment.

The role includes services to antenatal women, completion of the Healthy Child Programme for under 5s, enhanced programmes of care within school and community settings for the 5‑19 age group, breast‑feeding support and a strong element of leadership to the skill‑mix 0‑19 cluster team.

As post holder, you will be responsible for case management, assessment, triaging, planning and delivery of care for parents, children and young people using evidence‑based interventions, delivered in line with national and local guidelines and public health priorities.

You will be responsible for the operational overview of the Healthy Families Team allocated to a defined geographical population. The team will work proactively using a partnership approach to promote health and contribute to tackling health inequalities.

You will work with children, young people and families within the home environment, school or other identified community setting. A key aspect of the role is to work flexibly to meet the needs of the service as required.

• A full UK driving licence and vehicle for business use is required for this post; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
• Applicants will be required to pay for their DBS check; the cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), deducted from salary over the first two months of employment. Enrol for the DBS Update Service if desired (annual fee £16).
