Job summary

Hywel Dda University Health Board is seeking a Band 5 Wellbeing Practitioner based at Prince Philip Hospital, Llanelli, to deliver high quality telephone triage, coaching and assessment across all ages. The role supports families and carers and interfaces with GP services and local partners.

The post requires flexible shift work, including out of hours and weekends, within a bilingual NHS setting, contributing to a modern mental health access strategy.

Qualifications

• All-age service providing high quality telephone triage, coaching, advice and assessment.
• Information, guidance and support for families and carers as part of care.
• Shift-based work including out of hours and weekends with a multi-disciplinary team.

Responsibilities

• Provide 24/7 telephone triage and assessment for mental health; support safe outcomes.
• Work under the guidance of a Senior Practitioner on a shift roster.
• Contribute to development of the Single Point of Contact service with clinical links.
• Deliver care in line with organisational values and self-care options.
