Job summary

Hywel Dda University Health Board is recruiting a Band 5 Wellbeing Practitioner based at Prince Philip Hospital, Llanelli, to run a 24/7 telephone mental health triage service and provide high quality coaching, advice and assessment.

The post supports the Transforming Mental Health Agenda and works with GP practices, Welsh Ambulance, Local Authority, Police and third sector partners, with shift work including out-of-hours and weekends.

Qualifications

• Band 5 Wellbeing Practitioner role in NHS Wales.
• Experience providing mental health telephone triage and brief interventions.
• Ability to work within a multidisciplinary team.

Responsibilities

• Provide all-age high quality telephone triage, coaching, advice and assessment.
• Provide information and guidance for families and carers.
• Operate a 24/7 telephone line for partner agencies and maintain safe, effective care.
• Work on a shift basis including out-of-hours and weekends.
