Job summary

Hywel Dda University Health Board is seeking a Band 5 Wellbeing Practitioner for the Mental Health Single Point of Contact based at Prince Philip Hospital in Llanelli. The role involves telephone triage, coaching, advice and risk assessment to individuals with mental health needs, delivering a safe clinical decision.

You will join a 24/7 service with a multidisciplinary team, on shift work, and you may be required to work out of hours and weekends.

Qualifications

• Educated to graduate/diploma level in a related field (e.g., psychology, social care, occupational therapy) or equivalent experience.
• Knowledge of mental health conditions and related legislation (Mental Health Act, Mental Capacity Act).
• Experience in delivering therapeutic interventions and risk awareness.
• Ability to travel across the health board footprint and work flexible hours.

Responsibilities

• Provide high quality telephone triage, coaching, advice and assessment for individuals with mental health concerns.
• Offer information, guidance and support to families and carers; coordinate with partner agencies.
• Work under supervision on a 24/7 shift pattern with a multi-disciplinary team.
