Job description
Overview

Norfolk Community Health and Care NHS Trust invites applications for an Urgent Community Response Nurse. This is a 12 month secondment / fixed term contract to support urgent community 2 hour response for patients in their own home, with skills in palliative / end of life care, wound care and catheter care. The closing date is 15 February 2026.

You will be required to work over a 7 day working week including weekends. You should be hardworking, motivated, and possess excellent communication skills. You must hold a full and valid driving licence with access to a vehicle. You will support staff at bands 3 and 4, along with students and apprentices.

Main duties of the job
  • Work within the Urgent Community Response Team to prevent unnecessary admission to hospital.
  • Assess and prescribe care pathways for patients in crisis with long term conditions, frail & elderly, palliative/end of life care and rehabilitation needs to improve quality of life and independence, with support from senior colleagues.
  • Act as the patient advocate, facilitating choice and patient empowerment.
  • Provide evidence-based clinical/therapeutic interventions based on best practice.
  • Support early discharge from hospital within the Urgent Community Response Team.
  • Work with all healthcare professionals and statutory/non-statutory agencies to provide a seamless, integrated service.
  • Triage referrals on a rotational basis for the Urgent Community Response Team.

Job responsibilities

Main Duties & Responsibilities.As part of the Urgent Community Response (UCR) Team you will be required to:

  • Work within the Urgent Community Response Team to prevent unnecessary admission to hospital.
  • Assess and prescribe care pathways for patients in crisis with long term conditions, frail & elderly, palliative/end of life care and/or rehabilitation needs to achieve quality of life and independence with support from Senior colleagues.
  • Act as the patient’s advocate, facilitating choice and patient empowerment.
  • Provide evidence-based clinical/therapeutic interventions based on best practice.
  • Work within the Urgent Community Response Team to support early discharge from hospital.
  • Work with all healthcare professionals and statutory/non-statutory agencies to provide a seamless, integrated service to our service users.
  • Triage referrals on a rotational basis for the Urgent Community Response Team.
Clinical
  • Assess, plan, implement and evaluate all aspects of patient care and develop treatment/care pathways from a range of options.
  • Follow a holistic assessment; devise an individualised care pathway and adjust as required.
  • Be aware of Assistive Technology and utilise where required.
  • Assess patients’ holistic needs, communicating complex information to patients and carers regarding assessment, diagnosis, prognosis and treatment plan.
  • Adjust explanations for barriers to understanding (e.g., hearing or cognitive impairments) to gain understanding, consent and concordance.
  • Establish and maintain therapeutic relationships to enable engagement in care plans.
  • Liaise with GPs, Social Services, inpatient teams, allied health professionals and the voluntary sector to ensure needs are met and care coordinated.
  • Undertake risk assessments and act upon them appropriately.
  • Participate in multi-disciplinary/multi-agency meetings as appropriate (e.g., Gold Standard Framework).
  • Record activity data and clinical information on SystmOne daily.
  • Demonstrate dexterity and coordination when using equipment; maintain fine motor and mobilising skills.
  • Order equipment via electronic ordering systems within the team.
  • Obtain informed consent prior to interventions.
  • Adhere to universal precautions and infection control policies when exposed to bodily fluids or infectious materials.
