Job summary

Stockport NHS Foundation Trust is seeking a Caseload Coordinator for Urgent Community Response & Virtual Ward on a part-time basis (30 hours per week). This role supports caring for patients at home, coordinating caseloads and data, with weekend work required.

You will work in a busy MDT, use EMIS/Docobo, handle calls, and contribute to safe, effective patient care. The post offers a flexible working environment and a supportive team, with salary £25,760-£27,476 pro rata.

Qualifications

• Good general education including English and Maths.
• NVQ Level 3 or above in a relevant subject such as Customer Care, IT, or Business Administration or equivalent experience.

Responsibilities

• Manage the team caseload of clinical staff in UCR/HaH.
• Input and maintain data for patients and support accurate records.
• Answer telephone queries and pass information to clinicians.
• Maintain and monitor home monitoring equipment and databases.
• Participate in team audits and data collection.
