Job summary

East of England Community Health and Care NHS Trust is recruiting an Urgent Community Response Nurse to assess, plan, implement and evaluate nursing care in patients' homes, aiming to prevent hospital admissions and support early discharge.

The role involves face-to-face visits, 7 days a week service, with shifts 8am-8pm; you will work with GPs, social services and MDTs to deliver patient-centred care and use assistive technology as needed.

Responsibilities

• Assess, plan, implement and evaluate patient care in home settings.
• Develop individualised care pathways for patients in crisis or complex needs.
• Coordinate with GPs, social services, inpatient teams and MDTs for integrated care.
• Undertake risk assessments and act on findings to ensure patient safety.
• Record activity data and clinical information daily in the allocated system.
