Stockport NHS Foundation Trust is recruiting for a Caseload Coordinator for the Urgent Community Response and Hospital at Home teams. This permanent, part-time role (30 hours per week) involves coordinating caseloads, data handling and flexible weekend work to support patients at home and facilitate earlier discharges from hospital.

The post requires strong organisation, clear communication and the ability to work across disciplines, with a focus on patient safety and high-quality admin support.

Responsibilities

• Manage the caseload of clinical staff within the Urgent Community Response (UCR) and Hospital at Home (HaH) team.
• Gather, input and provide essential data for patients, helping to maintain accurate records and enhance the team’s ability to deliver care.
• Provide administration support and caseload coordination to support delivery of care to patients.
• Answer telephone calls from patients and relay information to clinicians.
