This is an exciting time to join our team, supporting us to care for patients safely in their own home avoiding unnecessary admissions and facilitating early discharges from hospital.
UCR/HaH is a multi-disciplinary team that provides a two-hour urgent response to patients in their own home that are experiencing a crisis. The service can clinically monitor patients remotely via Virtual Ward using enhanced technology.

We appreciate your interest in this opportunity and encourage applications from candidates who already have the right to work in the UK.
This role will involve managing the case load of clinical staff within the Urgent Community Response (UCR) and Hospital at Home (HaH) Team.
