DurhamEnable is recruiting a Triage Advisor to join Durham County Council's Connect to Work programme. You will be the first point of contact for referrals, carrying out assessments and signposting participants to appropriate services while coordinating with the Team Leader to guide people into the right employment pathway.

Responsibilities

• Carry out initial assessments to understand individual needs.
• Priori tise support and signpost to the right services.
• Guide participants into the appropriate employment pathway (SEQF or IPS) with our Team Leader.
• Build strong relationships with participants, families, partners and employers.
• Contribute to shaping initial support plans and ensure smooth transitions into the right support.
