Overview

A Vacancy at Derbyshire Community Health Services NHS Foundation Trust. Are you interested in being part of a responsive, innovative and dynamic Community Respiratory Team? We deliver a leading edge Pulmonary Rehabilitation Service across North Derbyshire area. You will be based at Walton Hospital and work across different Community Hospitals and community settings, including patient homes within the North Derbyshire area, including High Peak.

Responsibilities
  • Deliver Pulmonary Rehabilitation exercise and education programmes in North Derbyshire.
  • Undertake home visits independently to assess patients’ Home Pulmonary Rehabilitation (PR) exercises and provide modification where required.
  • Order equipment and refer to other services as required.
  • Provide ongoing treatment, teaching and instruction to referred caseload of patients.
  • Assess individual health needs and identify desired goals and outcomes of interventions.
  • Implement, modify and evaluate programmes of treatment without direct supervision.
  • Motivate and educate patients individually or in groups to enable patients to fulfil their desired goals.
  • Maintain accurate written and electronic records in line with professional and Trust standards.
  • Participate in continuous learning, reflective practice and share good practice with colleagues.
  • Comply with all Trust policies, procedures and local standards of practice.
Qualifications
  • Registered or qualified Respiratory Physiotherapist (Band 4 or equivalent).
  • Experience in community pulmonary rehabilitation, including home visits.
  • Strong knowledge of NHS standards, NICE guidance and local service specifications.
  • Excellent communication and interpersonal skills.
  • Ability to manage a delegated caseload within occupational policies and relevant codes of conduct.
