Job summary

A leading healthcare provider in Glossop is seeking a qualified Community GP to provide clinical leadership within a multi-disciplinary team. The role involves home visits, triage, and quality improvement activities. Applicants should be vocationally trained GPs with current GMC registration. The position offers part-time hours with a pay of £10,500 per session, dependent upon experience. This role provides the opportunity for a holistic approach to patient care in the community.

Community GP – Glossop PCN

Are you enthusiastic about providing holistic care for reactive and proactive care? Do you want a role which transcends the traditional boundaries of health and social care? Do you want an alternative to the current model of General Practice, where you can spend more time with the most complex patients?

Glossop PCN is looking for a Community GP to support our multi‑disciplinary team providing healthcare to patients in our neighbourhood. This role is being developed to integrate with the new neighbourhood aims and agendas.

Responsibilities
  • Provide day‑to‑day clinical leadership for the multi‑disciplinary team, including debriefing after visits if needed
  • Home visiting for clinically complex cases (e.g., end‑of‑life care, diagnostic uncertainty, complex prescribing decisions)
  • Senior clinical triage for the Acute Home Visiting service (currently 8 am‑6:30 pm, Monday to Friday)
  • Build relationships across the extended team, including social care, ambulance, social prescribers, community and mental health clinicians
  • Opportunity for involvement in service development as the team expands
  • Participation in MDT meetings
  • Provide holistic comprehensive assessment and enhanced advance planning
  • Maximise best care in the patient’s own home to reduce the need for hospital or care‑home admissions
  • Provide end‑of‑life care in line with the patient’s wishes
  • Engage in quality improvement activities
  • Offer opportunities for teaching and mentorship

Working Details

Location: 15a Pennine Road, Simmondley, Glossop, Derbyshire, SK13 6NN.

Working pattern: Part‑time, Job share.

Contract: Permanent.

Pay: £10,500 per session (dependent upon experience). Salary will be discussed during the interview.

Service hours: 8 am‑6:30 pm, Monday‑Friday. Number of sessions available varies from two to eight and will be discussed during the interview.
