Job summary

Riverside, Honor Lea in Lewisham, is seeking a Support Worker to work on a rolling rota at one of our supported services. You will provide person-centered care and support to customers, helping them to develop skills and independence while following local policies and safeguarding practices.

You will work as part of a team, engage with customers, and support them with tenancy related matters and access to community resources.

Qualifications

• Experience of working with vulnerable people in a support capacity
• Good communication skills and the ability to engage with and respect the needs of vulnerable people
• Excellent team working skills with a creative flair and ability to think outside of the box.

Responsibilities

• Carry out assessments to identify and prioritise needs
• Use SMART goal planning to provide needs led holistic support
• Create an environment that promotes opportunities for customers to develop, learn and enable skills towards independence
• Provide a comprehensive support planning service in line with policies
• Ensure customers are informed of their rights and responsibilities and are enabled to get involved in service via consultation and participation
• Set up and maintain customer files and accurate records of support provided
