Job summary

Home Group Limited is seeking an enthusiastic support worker to lead planning and coordinate our team to deliver person-centred housing support. This hybrid role blends service-based work with working from home, covering shifts in the community and in our services.

You will support customers with daily tasks, coordinate care, and work with multi-agency teams to safeguard and empower individuals with complex needs.

Qualifications

• Experience in supported housing is preferred.
• Level 3 Diploma in Care or equivalent, or working towards it.
• Experience assessing referrals and coordinating support.
• Ability to work independently, stay calm under pressure and be resilient.
• Willingness to work across services in the area.

Responsibilities

• Lead support planning and coordinate our Support Workers to deliver person-centred support.
• Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads.
• Support Customers with daily tasks like budgeting, cooking and attending appointments.
• Work with multi-agency teams to safeguard and empower our customers.

The practical bits

• 5 week rolling rota, varying shifts covering early mornings, days and evenings
• Availability to work waking nights and sleeps as required
• Hybrid role working in services and at home
• Although you'll lone work, we do get together regularly for training and team meetings.
• You need an Enhanced with barring list (we pay)
• A vehicle insured for business use (and a license!), we'll pay for any business mileage
