Job summary

Home Group Limited is seeking a dedicated support worker to help people with complex needs live more independently at Towers House Young People Accommodation in Consett. You will lead care planning, coordinate the delivery of person-centred support and work with a multi-agency team to safeguard customers.

The role requires experience in supported housing, a Level 3 Diploma in Care or equivalent, and the ability to act with initiative under pressure to empower young people aged 16–26 to move

  • Experience assessing referrals and coordinating support.
  • Ability to work on own initiative, stay calm under pressure and be resilient.

Responsibilities

  • Lead support planning and coordinate to deliver person-centred support.
  • Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads.
  • Support Customers with daily tasks like budgeting, cooking and attending appointments.
  • Work with multi-agency teams to safeguard and empower our customers.
  • Help customers build independence and prepare for their move-on into the community.

What you'll do

Requirements
  • Passion and experience in supporting others ideally in a supported housing setting and experience of working with vulnerable children and young people.
  • Level 3 Diploma in Care or equivalent experience (or are willing to work towards it)
  • Experience assessing referrals and coordinating support.
  • The ability to work under your own initiative, remain calm under pressure and have a resilient approach

The practical bits
  • You'll work a variety of shifts, early shift 7:30 - 16:00, Mid shifts 11:00 - 20:00 and Late shifts 13:00 - 22:00
  • You will be service based at Towers House Young People Accommodation in Consett.
  • Service based role working with males and females aged between 16 and 26 years old who are homeless and require support to develop their independent living skills.
  • Team meetings held once every 4 weeks (included in contracted hours)
  • You need an Enhanced with barring list (we pay)
