An NHS healthcare provider in Woolwich seeks a Clinical Support professional to assist the ADAPT team with patient assessments and care planning. The role involves developing care plans, conducting mental health assessments, and supporting the consultant during absenteeism. Ideal candidates will have relevant medical qualifications and experience in mental health care, benefiting from an established support system for professional development and training.
The ADAPT Pathway provides focused, therapeutic interventions to adults residing within the Borough who require care and treatment for anxiety, depression, affective disorders (without psychosis), personality disorders, trauma, dissociative disorders, eating disorders, autism spectrum disorders (ASD), and attention deficit hyperactivity disorder (ADHD). Care is offered to patients at CPA and Non-CPA levels.
The service aims to be flexible and accessible. The pathways of care aim to offer individuals choice and involvement regarding their treatment, actively encouraging their participation in the planning and implementation of their care.
A personalised care plan will be devised in consultation with the service user and their Carer/family where appropriate.
