A healthcare organization is seeking a Specialty Doctor in Frailty/Intermediate Care at Chepstow Community Hospital. This role involves supporting the Frailty team, managing community patients, and coordinating care with local GPs. The position offers opportunities for professional development and flexible working arrangements, aiming to provide high-quality care and reduce hospital admissions. Salary ranges from £62,117 to £99,216 per annum depending on experience.
An exciting opportunity exists as part of the Health Board's Clinical Futures model and redesign work within Frailty/Intermediate Care. The Frailty Service is a multi-disciplinary service within the Primary Care and Community Division, centred on providing people with care and/or treatment close to home, promoting independence via reablement teams. The aim of the service is to reduce unnecessary hospital admission by providing safe alternate pathways and minimising hospital stay by facilitating early and safe discharge.
The redesign of services for older and/or frail people means that you will be joining the Primary Care and Community Services Division at a once in a generation era, helping to develop a 'best in class' Frailty/Intermediate Care service model.

ABUHB is highly flexible in respect of career progression, for example the MRCP(UK). We actively support doctors who may wish to pursue a CESR route for specialist registration. Audit and QI projects are a requirement of the post and we have close links with Cardiff University should you want to be involved in research.
