Main duties of the job
  • Deliver high-quality, safe, and effective patient care as part of a collaborative team.
  • Promote a positive working culture by encouraging staff participation in service development and improvement.
  • Manage inbound referrals and respond appropriately to patient and family queries.
  • Update patient records with care plans during MDT meetings.
  • Coordinate follow-ups with the clinical team and liaise with hospital teams and practices regarding patient care.
  • Process patient discharges and ensure all care-related tasks are completed.
  • Report incidents, complaints, and escalations to the Service Manager.
  • Perform additional duties in line with NHS policies and legislative requirements as necessary.

Job responsibilities
  • Ensure the provision of high quality, safe and effective patient care
  • Work collaboratively as a key member of the team, promote a positive working culture, encouraging staff participation and involvement in developing and improving the service
  • Actioning inbound referrals appropriately
  • Dealing with queries from patients / family members
  • Updating patient records during MDT meetings with plans
  • Coordinating with the clinical team for any follow ups
  • Liaising with the hospital teams regarding patient care
  • Liaising with practices regarding patient care
  • Processing discharges once patients care has ended
  • Report any issues/ incidents/complaints/escalations to the Service Manager

Other responsibilities include, but are not limited to:

  • To undertake any other duties commensurate with the post and to react to statutory, legislative and NHS policy within the scope of the role

This position is to cover the below hours (10 hours per week):

Saturday 0800-1300

Sunday 0800-1300

Person Specification
Experience
  • At least 1 year -
  • Administrative skills and robust record keeping
  • - Ability to effectively communicate and resolve issues over the phone in a professional and courteous manner
  • - Coordinating and liaising with multiple stakeholders or individuals to meet specified outcomes
  • - Experience of working in Health and Social Care, or similar Public Sector roles
Knowledge and skills
  • - Good working knowledge of Microsoft Suite (including Excel, Word, Teams)
  • - General IT set up and systems
  • - Excellent written and verbal communication skills
  • - The ability to plan and organise own workload without direct supervision
  • -Excellent inter-personal skills and the ability to communicate and work effectively with a wide range of people from different backgrounds at all levels of seniority
  • - The ability to be innovative, objective as and logical in resolving problems
  • - The ability to prioritise and balance a range of competing tasks to deliver to agreed timescales
  • - NHS specific IT system experience (SystmOne)
  • - Understanding of NHS Community Services
  • - Experience dealing with difficult conversations / complaints
