Job summary

Derbyshire Community Health Services NHS Foundation Trust (DCHS) invites applications for a role in the Community Cardiac Team, supporting patients living with heart failure in Derbyshire. You will be part of a friendly team providing comprehensive administration and secretarial support.

The role handles remote patient contact via telephone, arranges appointments, processes post, scans, and liaises with GPs, secondary care and other services, while ensuring confidentiality and data protection.

Qualifications

• Excellent communication skills, both verbal and written.
• Professional "can do" attitude.
• Strong administrative IT skills and preferably experience of SystmOne/TPP.

Responsibilities

• Duties include being the first point of contact for the Community Cardiac Team.
• Assist patient contact mainly over the telephone, book appointments, send letters, scan, data entry, and process post.
• Liaise with clinical staff, GP services, secondary care colleagues and other health care services.
• Maintain confidentiality and data protection, prioritise workload and balance competing demands.
