Job summary

Change Grow Live in Peterborough is seeking an Outreach Worker to engage service users, assess needs and guide them to appropriate interventions in a supportive environment.

You will work with individuals with a history of rough sleeping and substance misuse, delivering screening, recovery planning, and onward referrals while promoting user involvement and partnership with colleagues and external services.

Qualifications

• Strong understanding of substance misuse issues
• Excellent verbal and written communication and IT skills
• Proactive with good time management and prioritising
• Good working knowledge of mental health interventions, services
• Collaborative team player able to work with diverse partners
• Adaptive to change prioritising service user needs
• Commitment to information governance and safeguarding
• Willingness to learn and develop professionally

Responsibilities

• Supporting service users from entry into the service and through their treatment/recovery journey
• Providing screening, assessment, recovery planning and onward referral
• Reducing drug and alcohol related harm to service users and the wider community
• Promoting involvement of carers, service users and the community
• Providing advocacy for access to partnership services
• Working with service users to support social reintegration and meaningful, purposeful lives
