Job summary

Change Grow Live in Sefton is seeking a Recovery Coordinator to assess and engage with service users, ensuring access to the right intervention at the right time in a recovery-focused manner.

You will provide screening, assessment, recovery planning, and onward referrals, working with partners to support social reintegration and post-care success for individuals on their recovery journey.

Qualifications

• Excellent understanding of drug and alcohol issues and experience of working within a related field.
• Strong verbal and written communication, with proficient IT skills.
• Proactive, able to work independently with good time management and prioritisation.
• Solid understanding of mental health interventions, services and best practices.
• Team player with ability to collaborate across professionals, agencies and stakeholders.
• Ability to navigate change while prioritising service users’ needs.
• Commitment to information governance and safeguarding.

Responsibilities

• Provide unwavering support to service users from initial contact through treatment and recovery.
• Screening, assessment, recovery planning and onward referrals.
• Deliver tailored interventions to reduce harm from opiate use, group or one-to-one.
• Develop recovery-focused plans including pre-detox preparation and post-care support.
• Collaborate with organisations and community to refer to mutual aid and networks.
• Advocate for involvement of family, service users and community.
• Serve as voice for service users in accessing partnership services.
• Enable social reintegration and promote recovery, resilience, peer support and self-determination.
