Job summary

Change Grow Live in Wirral is seeking a Recovery Coordinator to join our Non-Opiate team, assessing and engaging service users to access the right intervention at the right time. You will work within an integrated, person-centred care pathway and contribute to personalized recovery plans.

You will deliver 1:1 psychosocial interventions, co-facilitate group work, and collaborate with social services, mental health, housing and probation partners to reduce harm and support ongoing recovery for

Qualifications

• Experience engaging service users with substance misuse.
• Ability to coordinate with social services and health services.
• Willingness to work toward NVQ3 if not held.
• Understanding harms of non-opiate substances.

Responsibilities

• Provide bespoke psychosocial interventions for identified service users to engage in appropriate treatment.
• Deliver 1:1 PSI interventions and co-facilitate group interventions.
• Collaborate with social services, mental health, housing and probation to reduce harm and support recovery.
• Attend multi-disciplinary meetings and produce high-quality written/verbal reports.
• Engage service users and Peer Mentors in service development.
