Job summary

Change Grow Live in Mansfield is seeking a Recovery Coordinator to assess and engage service users, delivering recovery-focused interventions and coordinating care within the single point of contact team.

You will conduct triage, assessments and develop individualized recovery plans, ensuring confidentiality and accurate data reporting while upholding safeguarding principles and harm-reduction commitments.

Qualifications

• Experience of working with substance misuse service and harm reduction principles.
• Experience delivering services with high performance and quality standards.
• Accomplished written and verbal communication skills; strong IT and record-keeping capabilities.
• Experience and knowledge on safeguardingChildren and Vulnerable Adults.

Responsibilities

• Work as part of the single point of contact team in Mansfield.
• Engage service users with substance misuse issues and provide advice, support and recovery-focused interventions.
• Carry out triages, assessments and develop individual recovery plans.
• Ensure excellent customer service with a no-wrong-door ethos and timely data reporting.
• Be creative to promote access into treatment and continued engagement.
• Work with safeguarding leads to identify at-risk service users and reduce risk.
• Maintain confidentiality and obtain consent to share information.
• Support reception, duty cover and clinical duties as needed.
• Assist screening processes including DBST, urine and saliva tests after training.
• Carry out harm-reduction tasks such as Needle Exchange and Naloxone distribution after training.
