Job summary

Change Grow Live in Liverpool is seeking a dedicated Opiate Recovery Coordinator to join the Croxteth team. The role focuses on assessing and engaging service users to ensure timely access to appropriate interventions, working with partners across health, housing and social care to deliver coordinated care.

You will deliver assessments, risk and recovery plans, provide psychosocial interventions and harm-reduction advice, support OST, maintain records, and participate in MDTs.

Qualifications

• Experience delivering interventions for service users affected by opiate use.
• Strong verbal and written communication skills; proficient IT use.
• Proactive, independent worker with good time management and prioritisation.
• Solid understanding of mental health interventions, services and best practices.
• Team player with ability to collaborate with diverse professionals and stakeholders.
• Ability to adapt to change while prioritising service users’ needs.
• Commitment to information governance and safeguarding guidelines.
• Eagerness for continuous learning and colleague development.

Responsibilities

• Manage a caseload of individuals accessing structured treatment for opiate dependence.
• Complete comprehensive assessments, risk assessments, and recovery plans.
• Deliver evidence-based psychosocial interventions, motivational interviewing, and harm reduction advice.
• Support individuals on opioid substitution treatment (OST) with regular reviews.
• Promote recovery through access to education, employment, housing and healthcare.
• Facilitate regular key working sessions and monitor progress.
• Maintain confidential electronic case records per policies and data protection laws.
• Participate in multidisciplinary meetings and clinical supervision.
• Support planned treatment completion, aftercare and sustained recovery.
