Change Grow Live is seeking a Recovery Coordinator in Nottinghamshire to coordinate personalised recovery journeys for service users, including assessment, planning and participation in treatment plans. The role requires strong risk management and effective communication across teams.

This permanent, full-time position offers 37.5 hours per week, with a focus on delivering compassionate, person-centred care and working across multiple sites as needed.

Qualificações

• Understanding of drug and alcohol issues and experience in a related field.
• Experience managing caseloads within a CQC registered service.
• Commitment to multi-agency working and partnerships with other professions.
• Accomplished written and verbal communication with IT proficiency.
• Ability to update and maintain records accurately and meet deadlines for information submissions.
• Excellent time management and ability to work on own initiative.

Responsabilidades

• Coordinate personalised recovery journeys through assessment and support planning.
• Oversee participation in group programmes, courses, treatment and medical reviews.
• Continually manage risk and minimise harm while supporting service users.
