Job summary

Change Grow Live is recruiting for a Recovery Coordinator to join the Non-Opiate Team in Wirral. You will provide bespoke psychosocial interventions and engage service users to access the most appropriate treatment, delivering a person-centered approach with a focus on recovery.

The role operates full-time (37.5 hours per week) on a permanent basis, within an integrated non-opiate service. Collaboration with social services, mental health teams, probation and housing providers is essential.

Qualifications

• Extensive knowledge and experience in effective risk management planning and working with individuals with multiple disadvantages.
• Experience providing harm reduction advice and information.

Responsibilities

• Provide bespoke psychosocial interventions for identified service users, aiming for the right treatment at the right time.
• Deliver 11 PSI interventions and co-facilitate group interventions with trauma-informed care approaches.
• Work with social services, mental health, probation and housing providers to reduce harm and support recovery.
• Develop bespoke individual service user support plans for sustained recovery at the earliest opportunity.
• Engage Think Family Co-ordinator to involve parenting programmes and reduce intergenerational trauma.
• Participate in multi-disciplinary meetings and provide high-quality reports.
• Monitor and evaluate service delivery with service users and Peer Mentors.
