Job summary

Nottinghamshire County Council is seeking a dedicated Reablement Worker to join the Short Term Assessment and Reablement Team (START). You will help adults regain independence in their own homes, delivering person-centred care with dignity and respect.

In this hands-on role, you’ll support with personal care, mobility, nutrition, and medication, while collaborating with therapists and managers to ensure safe, effective care and clear progress updates to the team.

Qualifications

• AQCF Level 2 in Health and Social Care or willingness to work towards it.
• Full driving licence and access to a vehicle for home visits.
• Basic knowledge of safe working practices and dignity in care.

Responsibilities

• Support adults with personal care, mobility, transfers, nutrition, and medication.
• Encourage independence through tailored support plans.
• Collaborate with therapists and managers to ensure safe and effective care.
• Monitor progress and provide feedback to the team.
• Communicate sensitively and professionally with people.
