Job summary

Cheshire East Council is advertising ten permanent Reablement Worker posts in its Community Reablement Services. The roles support clients at home, helping regain independence after illness or hospital discharge and coordinating with Social Care and NHS teams.

You will undertake assessments, attend initial visits, and provide health monitoring and rehabilitation tasks while developing risk assessment and equipment provision skills.

Responsibilities

• Help people regain independence after illness or hospital discharge.
• Rebuild personal care and daily living skills in people’s own homes.
• Work with Social Care and NHS teams.
• Carry out assessments for long-term care needs.
• Attend initial care visits.
• Deliver same-day support through the Prevent, Reduce, Enable rapid response service to prevent hospital admissions.
• Carry out basic health monitoring such as blood pressure and temperature checks.
• Deliver low-level healthcare tasks and rehabilitation exercises.
• Develop skills in risk assessment and equipment provision.
