Job summary

Derbyshire County Council is hiring Enablement Support Workers for the Short-Term Assessment and Reablement Team (START). You will work with a multidisciplinary group to support people at home, promoting independence and dignity through personal care, medication assistance, and wellbeing activities.

You will contribute to assessments, help achieve therapeutic goals defined by clinicians, and record progress.

Qualifications

• Familiar with using smartphones/apps for recording information.
• Must have a valid driving licence and access to a vehicle for work.

Responsibilities

• Join a multidisciplinary team, supporting social workers, occupational therapists and health professionals to set goals that promote independence.
• Assist people at home with activities of daily living and medication support to regain confidence.
• Support therapeutic goals with occupational therapists and physiotherapists and promote use of assistive tech.
• Contribute to the assessment process and help monitor progress.
• Record progress accurately and flag concerns as needed.
