Job summary

Wiltshire Council’s Reablement team seeks an allocations officer care coordinator to support people to maintain independence. You will coordinate service delivery, assess needs promptly and help customers achieve personal goals.

Working with Occupational Therapists, care providers and hospitals, you will manage schedules, allocate resources and monitor outcomes, ensuring high-quality care and smooth transitions for vulnerable clients.

Qualifications

• Strong understanding of working with vulnerable people and delivering high-quality care.
• Experience coordinating and organising support services.
• Excellent communication, organisational and decision-making skills.

• Coordinate the delivery of the Reablement service.
