Wiltshire Council's OT-led Therapy Team is recruiting an experienced Occupational Therapist to support reablement in people’s homes. You will manage a complex caseload and undertake strength-based assessments to develop person-centred plans.

Working within a supportive team in a hybrid model, you will coordinate with customers, families, carers and partner agencies to promote independence and positive outcomes, with opportunities for professional development and progression.

Formación

• Qualification in Occupational Therapy is required.
• Minimum of two years post-qualification experience.
• Full UK driver's licence is essential.

Responsabilidades

• Manage a complex therapy caseload.
• Undertake strength-based assessments.
• Develop person-centred reablement plans to help independence.
• Coordinate care with families, carers and partner agencies and promote evidence-informed practice.
