Job summary

Wiltshire Council is recruiting an Allocations Officer for the Reablement team in Chippenham. The role coordinates the delivery of the Reablement service, ensuring customers are assessed promptly and receive the right support to reach individual goals.

Working with Occupational Therapists, Reablement Workers, hospitals and care providers, you will manage schedules, allocate resources and monitor delivery to maintain high-quality care. Hybrid working is provided.

Qualifications

• The successful candidate will have a strong understanding of working with vulnerable people and delivering high-quality care.
• They will have experience coordinating and organising support services.
• They will have excellent communication, organisational and decision-making skills.

Responsibilities

• Coordinate and ensure timely assessment and delivery of the Reablement service.
• Manage schedules and allocate resources to meet goals and service quality.
• Monitor service delivery, support safe hospital discharges and care transitions.
