Job summary

Wiltshire Council's Sarum Reablement team in Salisbury is seeking an experienced Occupational Therapist to manage a complex caseload and deliver person-centred reablement plans in clients' homes.

You will undertake strength-based assessments, coordinate support with families, carers and partner agencies, and contribute to service development while promoting evidence-informed practice. A full UK driving licence is essential.

Qualifications

• Qualification in Occupational Therapy.
• Minimum 2 years post-qualification experience.
• Full UK driving licence essential.
• Ability to visit clients at home or other settings.

Responsibilities

• Manage a complex caseload and undertake strength-based assessments.
• Develop person-centred reablement plans to help people regain independence.
• Coordinate support with customers, families, carers and partner agencies.
• Provide specialist advice, manage risk and deliver positive outcomes.
• Support colleagues and contribute to service development, promoting evidence-informed practice.
