Job summary

Hampshire County Council is recruiting a Reablement Service Practitioner to work across the Reablement service, including occupational therapy and the community response team. You will support people to live independently at home, using an enabling and adaptive approach to care.

You will triage referrals, complete holistic assessments and plan interventions with a multi-disciplinary team, while maintaining strong communication and safeguarding support.

Qualifications

• GCSE Maths and English at Level 2 are required.
• Experience in Reablement/Health or Social Care is preferred but not essential.
• General knowledge of the Care Act 2014 and Mental Capacity Act 2005 is preferred.

Responsibilities

• Triage referrals, complete holistic assessments of need and plan interventions with a multi-disciplinary team.
• Provide information and advice using a strengths-based approach to enable informed decisions.
• Build relationships with service users, their networks and healthcare professionals.
