Job summary

Wiltshire Council is recruiting Reablement Support Workers to help customers regain independence after hospital discharge. You will visit clients in their homes and work with occupational therapists to co-develop personalised reablement plans.

You will be on a rota basis between 7am and 10pm, 365 days a year, with hybrid working arrangements. A valid UK driving licence and access to a vehicle are required to travel to visits.

Qualifications

• Excellent communication and interpersonal skills.
• Ability to work as part of a team and independently.
• Flexibility to cover a rota across early to late shifts, weekends and holidays.

Responsibilities

• Support individuals in the home post-hospital discharge.
• Co-develop personalised reablement plans with occupational therapists.
